Our Patient Wellness Program puts emphasis on the management of chronic health conditions through a comprehensive set of preventative measures such as assessment and screening, one-on-one or group patient education sessions on diabetes, nutrition, and weight loss management.Our patient-centered approach is respectful and responsive, and ensures that patient values guide all clinical decisions.
