Based on Cerner’s Ambulatory EHR solutions, United Medical offers participating providers tiers of solution functionality, allowing our delivery network to provide a true community strategy to integrate all providers and staff members with increased efficiency to meet their needs to deliver quality care, and their growth progression from the value added.

Participating providers can access patient information from any location where Internet access is available. In addition, Cerner’s software enhances documentation, improves health maintenance tasks, provides follow up reminders, and incorporates other important functions resulting in lower hospital admissions and readmissions.

The partnership with Cerner has provided us with the ability to incorporate pharmacies, radiology, primary care, specialty care and lab info systems with patient portals and hospitals/ HIEs to secure our vision of an Accountable Care Organization that works to bring all aspects of the care team together for the benefit of the patient. It also enables the management team to focus on improving efficiencies at the medical practice level through the support of its population health analytics and practice management support, including regular training and billing support.

The outcome is a patient-centered community model where physicians are able to access immunizations, inbound labs, health maintenance, reporting, diagnoses lists, medical history, documentation and e-Prescribing at any time, day or night.

The complexity of the healthcare delivery system and the increased demands for patient outcomes and satisfaction has necessitated increased efforts at improving the coordination of care. This is especially the case for patients with chronic illnesses whose conditions require specialized care teams backed by a clinical integration program to address their acute and chronic needs.

Accordingly we have created a logical domain, with “One Patient-One Chart”, uniting our network providers and more than 325,000 patients.

Our implementation approach is based on delivery of Cerner’s Ambulatory EHR solutions, with a full complement of 24/7 technical support services as well as training services, both coordinated from our corporate offices in Bear.

This has resulted in the recent recognition of 60 of our ambulatory sites as having achieved Stage 6 designation of the HIMSS Analytics Ambulatory Electronic Medical Record (EMR) Adoption Model, with over 86 percent Stage 6 utilization within the network. Our implementation also includes our practice administration services, including our standard patient services as well as practice policy and procedures, and our compliance services. This has led to NCQA/PCMH certification of 70 of our practice sites, a 31.5 percent share and the highest coverage of NCQA certified primary care sites in Delaware.
