The behavioral health program is an integration of primary care services, mental health services and substance abuse services. For individuals to live successfully in the community and self manage their diagnosis and related conditions. The program is led by our Licensed Clinical Social Worker.
Our goals are to have individuals maintain stability, to receive medication management from PCP, collaborate with outside providers and connect patients to services, should they request/require them. Our Licensed Clinical Social Worker works with individuals via phone, and in person in the PCP office. We are easily accessible to assist with your needs. Programs that integrate behavioral health and primary care improve the individual as a whole. Improving the behavioral health and medical health of the individual, leads to a healthier life of success.
UMACO Behavioral Health Program is a participant of Delaware’s, START Initiative (Substance Abuse Treatment and Recovery Transformation). This program was designed to increase access of substance abuse and behavioral health services to individuals in need. UMACO is proud to be a part of this program and assist the individuals and community in obtaining services that they deserve.
